If schools are closed for an extended period of time and schools are unable to complete the universal screener, level 1 screener, or implement the intervention please complete the following steps when reporting data on the school corporation or charter school website:

  1. 1) State the grade(s) and subset(s) in each grade that WERE administered prior to the period your school corporation and charter school was not in session. Please state the dates your school was NOT in session. Grades 1 and 2 were administered the Universal Screener with each of the 6 subsets: phonological and phonemic awareness, alphabet knowledge, sound symbol recognition, decoding skills, rapid naming, and encoding. Grade 3 had six students that were screened upon teacher or parent request. Grade 4 had one student screened upon teacher request. School was closed beginning March 16th and did not reopen for the remainder of the school year.

  2. 2) State the intervention program(s) used, or your school corporation or charter school planned to use, during the previous school year. Orton Gillingham and Fountas & Pinnell Leveled Literacy Intervention

  3. 3) State the number of students who received an intervention, or planned to receive, during the previous school year. 23 kindergarten, 35 first grade, 28 second grade

  4. 4) State the number of students diagnosed with dyslexia up until the time your school corporation or charter school was in session. We have 4 known diagnosed students in the elementary.
